AOS Vaccination Supplement

Hi there,

Just trying to get a head start on what will be required for my AOS application and I'm a bit confused about the medical information. As my medical from my K1 application will still be valid I believe I'll just have to file the I-693A, Vaccination Supplement. I'm hoping to file for AOS by December so I was wondering if the I-693A can be done in Canada before I leave or if it has to be done by a civil surgeon in the U.S. after arrival?

Any advice would be appreciated. Thanks!

Your medical is valid for one year.

The I-693A is not completed by you, it is completed (here in US) by a civil surgeon. He/She will examine your vaccination record (assuming you have one) and complete the form - they will usually give you a copy and give you a sealed envelope to submit with your AOS paperwork/forms.

Yes, it must be done by a USA civil surgeon. Shop around for pricing .... cause some of them charge an outrageous amount!!!!

I didn't send in the I-693A for my own AOS either, and didn't receive an RFE. I sent in a copy of the "Vaccination Worksheet" from my K-1 medical exam.

myself as well, I was fortunate to receive a copy of the worksheet for my records when my medical was completed.

We did actually get the I-693A completed here in the US.. it took us some digging to find a Dr. that wasn't going to rip us off and it was an hr drive, but it was worth it for peace of mind for us. But like everyone else has said others have been successful without getting it done and no RFE's.. make sure at the medical in Canada they give you a copy of the worksheet (you will need it either for submission with the AOS package or the Dr. State side). In our case the Dr. didn't even want to take the CDN DR's word on the vacination worksheet and required us to give him the records of my vaccinations for the I-693A to be completed.
