I almost sent my DS-230 yesterday but I noticed that my mother(my beneficiary)

needs another police clearance. There's a 10 months gap since she reached 16 that she don't have police clearance. I overlooked this one and now I'm getting paranoid looking for another mistake.I have question regarding passport biographic copy.. does it need to be notarized ? On DS-230 #2 - asking for aliases and used name..I wrote her maiden name. I read some posting about it last night that it should be the married name or is this only for different cases that who just got married and in the process of changing their name status and lastly do I need to send photo of my mother. :help: Thanks

Apparently I have heard being RFE'd for having gaps on places you lived. But I hope not in your case.

No, just photocopy, need not to be notarized.

If her legal name in the Philippines is with her married name, then it should be married name. When you applied for her originally, did you use her married name? Bottom line, all of her documentations must be the same, unless you noted on "Other Names Used"

No, it's not part of a requirement for the DS230

DS230 q1. Should be her legal name. I would assume Kris Aquino Yap (married name) is what she is using

Questions 1 and 2 are simple.

Q1 is the legal name used by the applicant, which is also will be the one used on her visa. In this case, nanay used her mother's legal name using her married last name.

As for Q2, as it is clearly stated "if married, give maiden name" simply ask for maiden name.

@Aurea: Why was your DS230 returned?

BETTER CALL THE NVC TO BE SURE.MY DS230 WAS RETURNED TO ME BECAUSE OF THIS OTHER NAMES USED.

Hi Rina,

I believe for AUREA's case which I asked about last week, her DS230 was sent back because of question #2. NVC said it should have been the married name like the one on the passport. EVEN though it clearly states "If married woman, give maiden name". I tried contacting NVC regarding this and haven't been able to get thru. I am actually on the phone on hold now as I typed this! :( I hope I can clear this up and get a straight answer from NVC. My wife is sending her stuff this week (DS230 & required docs) and I want to make sure on this one. I will let you guys know what answers I get from NVC.

Thanks D&A. I was just surprised why someone will be RFE'd for that. Hope all goes well. Little things like this should not delay further processing of our cases. :thumbs:

Okay VJ'rs with the same scenario....I just got off the phone from State Department and before that I talked to NVC (took me all morning btw but finally got a answer). BOTH parties (NVC & State Dept.) agreed that for questions #1 and #2 on Part 1 of the DS230 application - if you and your spouse got married and then has gotten your passport afterwards (like for my case, I got married in the Philippines last Nov. 06 and then my wife later applied for a Phil. passport, got it and has her married name on the passport) then you will need to put your spouse (s) MARRIED NAME (as shown on the their passport) for BOTH questions #1 and #2. Basic rule according to NVC and State Dept. is that they will go with what's on your spouse(s) name on his/her passport. Moreover, this will also apply for Part 2 questions #26 and #27. Hope that helps a few here!!

Here one for us appliers through NVC, My step sons passports have 2 last names, however when I applied and did the I-130 I only placed the first of the last names ( the fathers name ). How will this affect the kids? Do I need to update the application? The dad's information here in the US is all with just one last name as they took the second last name away as we applied for all his stuff; Info on Dad in signature.
