DS-156K at Montreal

My fiancee is filing out her checklist today to send out to the consulate tommorow. Her checklist does not mention filling out the DS-156K to submit with the checklist. However the VJ Montreal embassy ( here ) indicates it as a required form. Is the DS-156K just required for the interview and not when sending in the checklist? I ask since the DS-156K required a copy of her Birth Certificate and Police Certificate. She is getting her Police Certificate later this week. We would like to mail off the checklist tomorrow without having to wait for the Police Certificate.

02/29/2008 - AOS/AP/EAD NOA's dated this date

04/15/2008 - Biometrics
