Packet 3 arrived, what do i send back ?

Hi,

We just got our packet 3 yesterday from the embassy in Bangkok and im not sure what we are supposed to send back to them. I know we need to send the two biographic data sheets and the checklist, but do we send the ds-156 and ds-156(k) also ? Anything else ? Sorry if this has been asked before, but i coudnt find anything when i searched. I appreciate the help.

Hi Karvee2005,

I do not know for Thailand but for most countries you send the checklist and the DS-230 part 1. Some countries also require a copy of the picture and data section of the passport. Good luck, hope this helps. :thumbs:

yes for canada as well ... i faxed them the complete DS-230 part 1 (they don't need part2, it is for K3) and the check list

then they sent me the interview date...

and they will get all the other forms from u on ur interview day .. so make sure u have everything ready... medical,Background check ..forms and everything they asked for in the check list!

DS-230 and the signed statement at the end of the checklist for sure - as the DSL-1076 instruction sheet and checklist tells you to do.

People who have been submitting these materials in person, rather than by mail, for the past 6 months or so have been reporting that 2 passport-style photos and a photocopy of the passport ID page are also being requested.

Wow Moonlight they accepted a faxed copy of the DS-230 form and checklist. We are going through Jamaica embassy and my fiance works on a cruise ship so we were trying to figure out how he was going to get the forms turned in, packet 3 will be going to his friends house in Jamaica and she is going to fedex it to me...if he can fax in the papers that would be great, if not we will just have to fedex the forms back to the embassy.

We are not to that stage yet but I am always thinking ahead.

yes ! US Embassy in Vancouver/Canada did accept faxed Checklist and signed Ds-230 I .. check with ur Embassy they might do the same..

I am going through London Embassy and on form IV15 it says:

Please complete and return the following forms to us straight away. Failure to do so will delay the processing of your application.

• DS-230 part 1

• DS-156 (in duplicate - do not sign)

• DS-156K

• DS-157

Do different embassies require different forms? Am I doing the right thing sending all these in the mail to them, regardless of wether they asked me to or not. Or are you supposed to present them in person at your interview?

Hi Johanna,

How are you doing? :D With the U.S. Embassy in Bogotá, Colombia we fax and sent the checklist and DS-230 part 1, through a courier service. Because there has been cases that people have faxed it but the Embassy said they did not received it. If I was you I would play it safe, fax and send it in, do both. :thumbs: I have seen they do this in other countries also with the exception of Canada and U.K. :yes:

Hi Miranda,

Different forms are used in different U.S. Embassy’s. But the forms you are speaking about are the same for most. I am not sure for the U.K. you have to speak with someone going through there but most countries you fax and/or send the checklist and DS-230 part 1. Once you go to the interview you take the DS-156 (2 Copies, 1 for Bank), DS-156K, DS-157 (Males Only), Birth Certificate, Police Records, Medical, Lab Works, etc. Hope this helps. :yes:

I should have stated in my post that I was writing specifically about the consulate in Bangkok, the consulate that is of interest to karvee.

Yes, different consulates do sometimes have different supplementary forms and differing preferences for what materials they want submitted when.

Congrats on getting your packet 3 I assume that you also received your interview date., you are now a step closer....

When US Consulate gets your case from NVC, your fiancee can turn in DS-230 part 1 and the I-93 "the checklist" and get scheduled an interview date. For some reason, when my fiancee turned in hers they also asked for 2 passport type photos and her House Registration document (tabian baan) and Thai ID.

The following forms & documents will be needed at the time of the interview: DS156, DS157, DS156K, I-134, Passport, Birth Cert, medical Exam, Police Cert, 2 passport photos, K-1 Visa fee (2) RECEIPTS from certain Post Office in Bangkok Area (will have applicant's names on BOTH receipts)

These 3 forms: DS3024, DS3025, DS3026 are needed at the hospital at which your fiancee will have the medical exam performed (correct me , anyone, if I am wrong)

As a side note & if anyone's interested :

My fiancee was not from Bangkok...so it a disadvantageous for her to get as much done as possible when she was in Bangkok. This is what she did in one day ....

1) Get medical exam at Bumrumgrad Hospital 9am , stayed until 1130am

2) Applied for Police Certificate (for her it took 2 weeks to complete)

3) Went to US Consulate to turn in DS-230 part 1 and checklist I-93 form (after 1pm), Note: I think the embassy has changed this procedure, the forms must be submitted via email now. See website from BKK embassy.

4) Went to a nearby Post Office across from Embassy to pay the K-1 visa fee ($100) and was given a 2 part receipt with hernames on them.

Yes this is true, i recieved two seperate papers asking for that stuff. By the way, thanks for all the replies. :)

Point of clarification - that instruction sheet and checklist is form DSL-1076. The 'I-93' is 1-93, January, 1993.
