Help need to locate form 169

If anyone could post a link to the form 169, I would greatly appreciate it. I need this form for the K-1 visa but I am not able to locate it. Thank you in Advance

Go to http://www.visapro.com/US-Immigration-Forms/DS-169-Form.asp. Visapro has all the forms needed for the visa process. And it's free. Be sure to watch the expiration dates for each form you get.

My observation is that the OF-169's can tend to be country/consulate specific, and any sample is representative of what you will have to complete, but you will get the actual form in your P3. Or, you can check the consulate website for your SO's country.

Form OF-169 is now DS-2001. Different consulates may use consulate-specific versions of the form, so try the appropriate consular web site first then if no luck use Google.

The embassy website for Colombia does not have the OF-169. It will be in your Packet 3. A few people in the Colombian Club thread have a scanned copy of the OF-169 for Colombia--you might want to post there for Colombian-specific information. Someone might be able to email it to you. I do not have a scanned version--we just used what came in Packet 3.
