Please remember to include your case number, date of birth, spouses name and date of birth in your email

I didn't include my date of birth after copying a suggested email from the forum and I waited weeks for a reply, eventually I realised something must be wrong, and asked for advice, someone here kindly told me about including the date of birth, so I resubmitted my email and recieved a reply within 3 days.

Here is exactly what I wrote - you do not need to include the form (this is discussed in the NVC timesavers thread). Hope this helps They responded accepting the choice of agent within a few days.

Dear Sir or Madam:

My name is Michael xx, my visa case number is MTL2006xxxxxx.

I do not have my DS-3032 and I would like to appoint my wife as my Agent of choice to receive all correspondence from NVC.

Her name and address are:

All mail from the U.S. Department of State concerning my immigration visa application should be send to the address above. Thank you for your help.
