b1/b2 visa sample form?

my mom has a good friend who is going to apply a visitor visa w/ her son maybe 2008 of april. and she would like to see a sample form to fill out. anyone know where i can see it? b'coz next year her and her son wanted to visit san diego for a week or two, and i would let her and her son stay in our home. anyone knows? and is there something else i can do for their visit?! or visa? i would really appreciate it many thanks! anyways their from Philippines :unsure:

for tourist visa, believe you would use the DS-156 and DS-157. here's a link to visitor visas.
