Timeline

Beginning October 1,2014... St lukes will be collecting $245 medical examination fee for adults... Children's fee did not increase though... Just sharing.

Is this due to an exchange rate change? It is payable in pesos only, so that would affect it.

$245 is $245 no matter the exchange rate... and yes payable in pesos only, of course they don't flex the rate daily either.

Don't mean to change the subject too much.... but is it ok for my fiancé to do her medical 3 weeks before the scheduled embassy interview?

Embassy website says:

Applicants are advised to have their medical examinations done 5-7 working days prior their interview appointment at the U.S. Embassy in Manila; failure to complete the exam within the prescribed time frame may result in significant visa processing delays.

So I wasn't sure if doing it 21 days ahead was ok.

Yes you are fine to complete the medical a few weeks early. In the embassy instructions letter they add the words - "at least" 7 days before

USEM prefers that the medical be completed in time for your SLEC paperwork can be match with your file and available on the day of your interview. If you miss that opportunity and show up for the interview and your SLEC results are not available then you will be delayed. Your timeline of 2-3 weeks prior is a good plan, hence plenty of time for your SLEC results to reach USEM.
