Can anyone help me with the letter of intent? I am the petitioner and on the example of the intent letter does the section of the US CONSULATE, does this pertain to both my fiance and I or just him?

The address is not required for the letter of intent. The addresses are just a formality, and if you wish to include them then you would use the Dallas lockbox address. This is all that is required for the letter of intent:

Dear Sir or Madam:

I, [YOUR NAME], do hereby state that I am legally able and willing to marry [FIANCE'S NAME], and intend to do so within 90 days of my/her/his arrival into the US using the K-1 visa.

Sincerely,

[sIGN AND DATE]
