I got married and changed my name to my husbands new name.

My tax returns for 2013, 2012, 2011 have my maiden name.

My paystubs/job offer letter/ has my new name.

When filling out the I1864 where is says sponsors name - family name, do I put maiden or married?

Do I fill out everything with the maiden or married name.

My i-130 has my maiden name.

What name do YOU consider to be your legal name--married or maiden. If you want to use your married than do so. You are including a copy of the marriage certificate anyways and that is considered a legal document for changing your name. The key is here that you got married. Before you were not married and therefore were using they name you went by at that time. Now you are married and can choose to use your married name or continue to use your maiden name. Your choice. Just fill out the form in that name.

Good luck,

Dave

As the petitioner I would use your legal last name. If you have not changed your SSN you may want to use your maiden name, but for the petitioner, it's not that big of a deal. They can figure out who you are. ;)
