Questions about Adjustment of Status

Hi!

My fiance and I are getting married on Friday and are starting the Adjustment of Status documents. I have a few questions:

1) For the I-864, what type of proof of income do we need to send in if any? I just started my job (I was a graduate student last year) so my 2013 taxes don't reflect my current income. We had my dad co-sponsor for the Visa. Should he co-sponsor for this too?

2) I'm a little confused on who the Cover letter should be from/signed by. Visa Journey's guide says the "petitioner" For an adjustment of status does that mean the US citizen? Or does that mean the Immigrant?

3) He had his medical exam overseas before his interview. Do we need to submit proof of that or do they have record of that?

4) His birth certificate is in Spanish, do we really need to translate it? If so does it need to be notarized or something?
