Hi :)

1. Yes, use your married name

2. What I did is I put the year starting from January 2009 to Present then the address

3. I put my current address in the Philippines on that field.

so as for my case can i just write january 2009 as well up to present on the 2nd question?

thanks

NO, you write the month and year you lived there regardless of how long it has been. If you have only been at one address your entire life, you start with the month and year you were born in the 'from' category and write "present" in the 'to' category.

The other answers stating otherwise are not correct.

I assumed that they had moved there Jan 2009... Perhaps wrongly.
