There is a Letter of Intent template at VJ example forms: http://www.visajourney.com/content/examples

You can use the address that you are going to send the I-129F petition to, and it has been mentioned here too that putting no address will work as well. In the latter, you can simple use "To whom it may concern". Either way will work.

Put the address you send the I-129F petition to, OR put no addresses at all. They are not a requirement.
