1. For Mother's maiden Name:

Should we put her last name before she got married or her middle name before she got married?

2. If the Answer is 'None'

Should we leave it blank or indicate None?

3. I'm not sure about the exact dates of our residence. Would that matter?

Maiden name is whatever her legal name was before she married.

Never leave answers blank. Put none or N/A.

Can't help you on #3. Really don't know. Sorry.

1) Maiden name is her surname before marriage.

2. If the answer is NONE, write NONE just as the form instructs (if none, so state)

1. What should my fiance put in the Applicant's Last Address outside the United States of more than 1 year? She was born in the Philippines and went to the US in 2000. Should we indicate that?

No, they are asking for last address OUTSIDE the USA (this form seems biased toward a USC answering.. weird), for your fiancee I would say N/A for that question. I just checked my wife's G325A and that is exactly what we did N/A for that question.

No, they are asking for last address OUTSIDE the USA (this form seems biased toward a USC answering.. weird), for your fiancee I would say N/A for that question. I just checked my wife's G325A and that is exactly what we did N/A for that question.

We got confused there while filling out the form earlier.
