Form I-864 Question

Hey guys a couple question on this form

- part 2 question 2a - Is this my foreign spouses' mailing or my mailing address here in the US?

- part 2 question - Vietnam is not listed on the dropdown options should i select "Unlisted Entry (Any foreign country not listed")

- part 4 question 7 - I was born in a village with no town or city name, should I leave this blank and enter the name of the province?

- part 6 question 5 - It is asking for my current individual income, do I just enter my annual salary or should I also add in bonuses as well?

- part 6 question 13 - I have filed for an extension for the 2013 tax year because I'm applying for an ITIN for my wife, should I check this box? It is also asking whether or not I have attached a transcript or copy of my Federal Tax return for only the most recent tax year.

Mailing address of your foreign spouse (beneficiary or the Principal Immigrant).

- part 2 question - Vietnam is not listed on the dropdown options should i select "Unlisted Entry (Any foreign country not listed")

You have to print this form out anyway. So write the country (Vietnam) in the country box. I had to write in the Street Number and Name as I could not fit it into the box (there is a word limit).

- part 4 question 7 - I was born in a village with no town or city name, should I leave this blank and enter the name of the province?

I guess the City or Town box will have the name of your Village. In the box for Street Number and Name you could write N/A.

- part 6 question 5 - It is asking for my current individual income, do I just enter my annual salary or should I also add in bonuses as well?

This is the Adjusted Gross Income as per your 2013 IT return. Now looking at your next question since you have filed an extension for 2013, I am assuming you will be submitting 2012 IT return with this AOS. In that case, you should put the Adjusted Gross Income from the 2012 Tax Return or Total Income Per Computer from the 2012 Tax Transcript

If you are only submitting the 2012 Tax Transcript then check this box. On the other hand if you are submitting 2012, 2011, and 2010 Tax Transcripts then you should check box 14 and provide the last 3 years Total Income info in 13a, 13b, and 13c. In any case you should also send them copies of the 2013 Tax Filing Extension form, and also copies of all the W2+1099s.
