do I need to file i-693 for aos?

Hi everyone, we are about to file for adjustment of status. I was wondering if we still need to include i-693? I have done my medical at st lukes late november 2013. My vaccination record says: vaccine history incomplete, applicant may be eligible for blanket waivers because vaccinations not medically appropriate as indicated above

Unless you had a medical issue (not vaccinations) you should be able to make a copy of your DS-3025 to send in with your I-485. I suggest you print out a copy of page 4 of the I-485 instructions, highlight section 7c and staple it to the copy of the DS-3025 - there are some adjudicators that don't seem to know the I-693 is not required with AOS for the K-1, others have been doing this without issues.
