Just filled this out online and while it creates the barcode on the first page fine, the one at the end (on page three) looks a complete mess and certainly unlike any barcode I've seen before - is this right?!

(Had a look at the example forms and they don't seem to include it.)

Yup, thats right. It generates a 2 dimensional barcode that actually contains all the information you entered into the form. Why, they go through all that instead of having you fill out the form and submit it electronicly, i have no clue... use of our government dollars at work again!

but yea.. anyway, that static like gibberish at the bottom barcode is correct =)

I don't think Page 3 is essential to the form. It's just to let you know that some answers may have been blanked, such as "none," because it could possibly be someone's name. No bar code needed for this page.

Thanks. Most of these forms just seem like a way to waste/collect dollars!
