Why would you pay the Hospital anything?

J1 is usual for internships, not B1.

It was basically a clerkship program and it has a fee so, I paid and everything and the hospital requirement was B1 but for now that isnt the issue my problem is I never met him and I have pictures from the engagement ceremony from back in 2008 that I had with his mother and sister, can I use those pictures and all the evidences of online relationship and phone calls?

There are 3 aspects.

1. You need to show why he can not visit you, presumably reports from his medical team.

2. Why you can not visit him, might want to try one more time.

Presumably there is nowhere else you can meet? He is housebound?

3. Then there is the issue assuming you get past that hurdle of the interview. Could be seen that the motivation is more on moving to and working in the US.

he needs everyday 12 hours dialysis no way he can spend even a day in flight traveling it takes about 2 days to reach Pakistan or any country nearby plus he has dialysis set up in his room as his father is a doctor and they have the record of my past declined visas, so they know I tried because he said on my face why didnt your fiance applied for you

he has congenital chronic renal failure. He was born in Pakistan and was immediately taken right from the hospital to UK, he lived there for few years and then moved to USA. He never came to Pakistan since he left

Me and my fiance were engaged through parents back in 2004. We were too young to accept the relationship we never talked till 2010 and now we talk 24/7 and are in Love, unfortunately I could never meet my fiance because of his medical issues he cannot travel at all as he is suffering from Chronic Renal Failure and needs everyday 12 hours dialysis.

YOU can travel.

If he traveled he would die

Then why pursue a visa that would require him to travel to the US?

we have proof of online conversations, facetimes, pictures taken through webcam, presents he sent me on different occasions but not even a single picture together though I have few pictures with his family as his mother came multiple times.

Is this issue gonna affect our case?

Yes, you will be denied.

We already submitted a waiver from his doctor with the I129F application?

But you CAN travel.

There is no harm in attempting the waiver. If you get denied for the waiver, then you will simply have to find a way to meet in person.

Have you put any thought into the fact that you are cousins and if you were to have children it might be likely they will also have this condition? I'm not judging you in the least, but I'm sure as a doctor, the thought has crossed your mind.
