starting for fill up the D-160 form and scheduling for medical

hello all VJ's members,

Here i am again, asking again for help.

yesterday i already paid my visa fee at BPI and now im starting to fill up the D-160 for scheduling my interview. But when im almost done, i review my data and i saw in this part of TRAVEL INFORMATION this type of question

01 02 03 04 05 06 07 08 09 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25 26 27 28 29 30 31 JAN FEB MAR APR MAY JUN JUL AUG SEP OCT NOV DEC im confuse what is mean., Is it they asking when im plan to go to US or going for interview.

In medical form also asking, what can i put
