need urgent advice please

I need to know when at the time to apply for a U.S. passport for your child with the parent living in their country of origin, of course after having completed the entire process and obtained citizenship , the father of my child is in complete agreement , could share with me their experiences? . The officer at the post office told me he had to fill out a form and have it notarized, but that type of notarization? our process was through the visa k1 and k2 .... Thanks guys!!

The father will need to fill out a ds-3053. Here is the information from the state department's website.http://travel.state.gov/content/passports/english/passports/under-16.html#consent

He can get it notarized at the us embassy in his country of residence. I believe the cost is usd 50.

Is the U.S embassy the only place to notarize the form? Or he can use a private notary?

Generally most places require an American notary. The only people who can notarize (abd be recognized) outside are diplomats. There is a process of legalization and apostiles for recognizing foreign documents. Those aren't really easy.
