How does it read on the birth certificate? Example mine is first name middle name last name in that order. I also 4 names if your middle name 2 names make sure both names go in the middle name space. Hint: if there is a space(-), make sure you put it, or if 2 seperate names make sure it is separated properly, hint 2 a middle name can be 2 names, it is just not as common in USA as in foreign countries is all.. Remember has got to be exact as your birth certificate.

My fiance uses five names. We listed his fifth name as his surname and fit the other four in the other lines. They can be handwritten if they don't fit. There was a spot for other names, and we wrote out his names in order on this spot. When we got the NOA 1 his name wasn't correct, so I sent in a letter to correct it. The now difficult part is he uses only the first two names in his professional and personal life but all his paperwork has his full name. If I change my name, the second name is what I'd use as my last name, but I'm not so sure how that will work..we'd then officially still have different surnames. It makes it crazy I know. His passport has all five names on it, so we tried to be consistent.

You have to be consistent exactly. The name on the form has to match the passport, application, etc.

So put your first name as the first name, the next name is your middle name and both of the 2 last names as a last name with a space.

It all has to match. You cannot change it until you get over here and apply for citizenship. Then you can change it or shorten it as you go.

middle name is exactly that, "middle name", is any of those names your middle name? if not then it is up to you what to put in there, you can put something or nothing, since these are patronomic names and not middle name you don't have to put anything in there if you don't want to, just remember that this name will always be on all your immigration paperwork in the future if you put something in there.
