I-134 attachments

help me pls what are the attachments for FORM I-134 ?

because its already mixed up when i go to photocopy center i forgot to put marker on it or numbering each paper

hi when i checked the copy of my I I34 affidavit of support there is no attachment its only 2 pages and the last page is about your fiance employment,dependants,etc.and oath or affirmation of sponsor then your fiance signature also date.

just dont forget a copy of your most recent tax return should be 2013

I-134 is two pages and it has to be an original with sponsor's signature. Only other documents required by USEM are copies of the most recent tax return and W-2 showing gross income that meets or exceeds the I-864P poverty guidelines.
