after 10 months and 20 days of ap and then asked by embassy to renew medical and police report they sent me this letter with passport

they sent me my passport today with this letter , i dont know what i should do now

i dont know why they made me in ap for 11 months and renewed my medical and police report last week if they are not sure about our relationship and i dont know what we will do now((((((((((
