Dear all, my GC will be expired on may 15, 2014 so i have 90 days start from 16 march til 14 may 2014?

Right now i am going to fill the form but i have some question :

Part 1 :

1. other names used >>> should i write it because my passport still using my first name & maiden name ( My GC is already use my husband last name )

2. Alien Registration number >> is this from USCIS Letter when they sent a letter for approval of my GC with A- xxx-xxx-xxx

3. Mailiing Addres is the same address with Physical Address, so should i write it down too or leave it blank?

Part 2 :

Waiver request filing >>> which one for the answer i am confuse

Part 3 : About my husband

1. Where can i find his Alien registration Number?

Part 4 : Children >> we don't have any children but he does from previous marriage so i just leave this part blank

Part 5 : Blank / No

Part 7 : Preparer is myself or should be my husband?

For sworn affidavit support letter from friends, must be notarized by themselves going to a lawyer to do notarized or we have to do it after they write & sign the letter?

so is there any interview for ROC? because some people said no interview except if you have some special case. Do i need another medical checkup?

That's all & thank you in advance for your help, really appreciate :)

Hi.. I am not an expert so would recommend that you seek some more advice

Based on your questions, I am assuming that your Husband is a US citizen.

Part 1 :

1. other names used >>> should i write it because my passport still using my first name & maiden name ( My GC is already use my husband last name )

Ans :- Yes you should write it in section 2.a,b and c

2. Alien Registration number >> is this from USCIS Letter when they sent a letter for approval of my GC with A- xxx-xxx-xxx

Ans:- Alien registration number can also be found on your Green card, and yes it starts with A-XXX

3. Mailiing Addres is the same address with Physical Address, so should i write it down too or leave it blank?

Ans:- If bith addresses are same, leave Mailing address blank.

Part 2 :

Waiver request filing >>> which one for the answer i am confuse

Ans:- Waiver is filed when you are going through a divorce or separation (By any other means). I believe that you and your husband and Joint filing. In that case No need to check anything under Waiver Request section.

Part 3 : About my husband

1. Where can i find his Alien registration Number?

Ans:- I dont think you need his alien number, so leave it blank

Part 4 : Children >> we don't have any children but he does from previous marriage so i just leave this part blank

Ans:- I think it shuld be left blank, however will seek some more advice.

Part 5 : Blank / No

Part 7 : Preparer is myself or should be my husband?

Ans:- I think This is filled when someone else other that you two is filing the papers for you (For example an Attorney).

I think you can leave i blank.

As it says on the form, 'Other Names Used (including maiden name)', you would list your previously used maiden there.

2. Alien Registration number >> is this from USCIS Letter when they sent a letter for approval of my GC with A- xxx-xxx-xxx

The A# is on your green card.

3. Mailiing Addres is the same address with Physical Address, so should i write it down too or leave it blank?

As it states on the form, 'Mailing Address (If different than Physical Address)', if your mailing address is not different, then you leave it blank.

Part 2 :

Waiver request filing >>> which one for the answer i am confuse

You are joint filing with your spouse, yes? Then you would not need a waiver to file without him.

Part 3 : About my husband

1. Where can i find his Alien registration Number?

If he is a USC, then he does not have an A#.

Part 4 : Children >> we don't have any children but he does from previous marriage so i just leave this part blank

Yes, the form instructions state to leave anything that is N/A or none blank. You can also list his children and check the box that states the child is not applying with you.

Part 5 : Blank / No

Answer the questions, Yes or No.

Part 7 : Preparer is myself or should be my husband?

No one fills in Part 7, unless you hired an attorney or third party to fill out the form for you. You and your husband sign in Part 6.

For sworn affidavit support letter from friends, must be notarized by themselves going to a lawyer to do notarized or we have to do it after they write & sign the letter?

They do not need to be notarized They are not even required evidence. If you have plenty of evidence to prove your marriage is real, then no need for affidavits.

so is there any interview for ROC? because some people said no interview except if you have some special case. Do i need another medical checkup?

Sometimes people are randomly chosen to interview, or get interviewed because there is doubt about the validity of their marriage. Interviews for ROC do not happen that much, maybe around 10%.

That's all & thank you in advance for your help, really appreciate smile.png

You really should check out the example form here on VJ > http://www.visajourney.com/examples/INS-Form-I-751.pdf

You can file for ROC starting Feb 14, 2014, up until your green card expiration date. Calculator > http://www.uscis.gov/sites/default/files/dateCalculator.html
