Surname in Birth Certificate is different from surname in passport.. HELP!

Timeline

I am applying for a K-1 visa soon. I got my packet from the Tokyo Embassy today!

I'm a Filipino citizen living in Japan. I have been living here

for 9 years since I was 11..

That being said, I have no access to my birth certificate, my mom does.

I just found out that my surname was late-registered, and thus it's not a valid

document. I know I need supporting documents such as baptismal certificate,

school records, prenatal records...

What other documents do I have to submit?

Is an affidavit from relatives and friends mandatory?

Also, my father's name is not included in the late-registered birth certificate..

What should I do? My parents have been seperates since I was a kid..

Please help!

I really don't know what to do with my situation right now.

I live in another country which makes it really difficult.

My passport, driver's license here, school records here in Japan are all in my mother's

maiden name... are those documents sufficient or am I required to

submit documents that are dated around the time of my birth/childhood?

We really want to get our application approved so bad..

My mother have the same situation as yours, you may secure an "affidavit of two disinterested person" that will testify your surname, the two person should be senior citizen and have enough knowlege about your family. I did not go to province to file it in person, what I did is I google it and gave them a call, they assists me and were very helpful. You must have a copy also of marriage certificate of your parents as your supporting documents.

You need a lawyer for this because they may require you to publish it in news paper. Try to call the municipality that handle your birth certificate they can help you more on this.

I have the same situation as Baby Gee.

@Baby Gee - what did you do to correct your situation?

@OF and LG - what has happened to your mother's case? Did she use the 1st or 2nd registration? How long did the process take?

@Kristinebelle - did the local civil registrar disregard your first registration so you can use the late registration?

Please help!
