Ds 156 form HELP! I worked as an OFW in Hong Kong before should I put under ADDITIONAL VISA ISSUANCE my working visa in HK?

I am not in the same situation as you are....but just wanted to share my unsolicited observation of the DS 156...as I had filled up the ds156 form and I have here with me a photocopy of my DS 156 (2 originals were passed during my k1 visa last July 2013)..

No.30. Have you ever been issued a us visa? Yes. No

When?

Where?

What type of visa?

Additional visa issuances?

So I answered.....yes...14 feb 2012....Manila....B1/B2....04 April 2001 Manila B1/B2

No.31 have you ever been refused a us visa? Yes No

When?

Where?

Additional visa refusals

No.32. Do you....etc etc.

My k1 visa is approved and have my visa on hand so I can say my guess is right and think that I filled up the form correctly.

In conclusion, the additional visa issuances in No.30 is only asking for additional US visa issuances.
