I think I almost have this all set to go, just a couple questions:

1) I read about there being a cover sheet for this. Where do I obtain this?

2) Is there a fee for this, where can I find that information if so?

3) Will a deed of our house be enough proof or should we send more (don't want to overdo it)

Thank you

I just came across the fee I asked about. This is horrible timing as we just closed on our house, we should have looked into this more but didn't.

I'm assuming I HAVE to file this now? What happens if I do not? Will I be deported? Just wondering if there is anyway around this for the short term?

There's really no way around it, you MUST apply for ROC. I added cover sheet to my package, it helps especially if you have a large one. Some people even put labels on each of them as well. I'm sure there are plenty of examples on this forum but I can post mine too if you need it. Always over do it if you can, it helps later in case you have to scramble to find more evidence in a much shorter amount of time. Again, there are plenty of examples of evidence here.

The fee is listed in the form instructions and USCIS website. > http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=f858d59cb7a5d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=db029c7755cb9010VgnVCM10000045f3d6a1RCRD

Yes, you HAVE to file within the 90 days before your green card expires. > http://www.visajourney.com/content/751guide

A single deed to your home is not enough evidence, not even close.> http://www.visajourney.com/forums/topic/220440-i-751-evidence/

There is a cover letter sample in the example forms> http://www.visajourney.com/content/examples
