I just found out that USCIS requested our file back from NVC on Friday. They are unable to give me a reason why, because the file hasn't made it back yet. Has this happened to anyone else before and should I be worried? Any advice or comments would be appreciated.

Thanks.

NVC did their checks and found something missing or wrong or incomplete and found the uscis approved it and should not have. So they send it back for corrections.

I do not think you need to worry about it. Just another delay in a long process...

Well I know it happened recently. They had filed a K3 and cancelled the 129F and it started a chain reaction. USCIS asked for file back.

Mind you they already were done at NVC waiting on case complete.

Finally after 2 months of USCIS and NVC not knowing why or what file was sent back missing all NVC documents.

So they had to redo everything. Now they are waiting on NVC to review and case complete.

That is an extreme case. Hopefully it was just a forgotten document or sign off for you.

That is a little more disheartening. The reason we filed the i=130 is because my wife had to leave the country to go handle some family matters and her travel documents weren't renewed yet. (The first one had expired after one year.) Therefore our i-485 was considered abandoned. We have waited a long time with USCIS and I had to go to Russia to celebrate our two year anniversary last month. Crazy! I really do hope that it is just some simple clerical issue.

Just got the letter from NVC it states:

NVC returned the petition to the Department of Homeland Security's U.S. Citizenship and Immigration Services (USCIS). Please direct any further inquiries to USCIS.

It doesn't give any reason why it was sent. Is this normal or should I be freaking out more than I already am?

No, it means you now need to ask USCIS. Don't panic until you know you have a reason to. Delay is not denial. If the NVC had wanted to deny your petition, I'm sure they would have done so.
