2Go Delivery for Chilld's Passport/Visa

US Travel Docs website says...

If the applicant is under the age of 16, the following documents are required:

- An original, signed letter of authority from either of the applicant's parents

Is this letter of authority required if it's one of the parents receiving the delivery? If so, does anyone have a sample so I can have my fiancee prepare one? Any experience/advice is appreciated. Thanks in advance VJers.

the child needs to be present when applying for the passport and one parent or both parents need to be there too. If one parent can not be there that is when they fill out the form on the link I gave you and have it notarized. Or if you have custody papers making you the sole guardian then you are fine.

I don't see the need for any letter if the parent, in your case it's only your fiancee, is the one who will receive her child's passport with visa.

If another person will receive the passport, your fiancee will have to make an authorization letter for that person with attached photocopy of your fiancee's valid ID and that person must present his/her own valid ID too.

To whom it may concern:

I hereby authorize my relative/friend, (name of relative/friend), to receive on my behalf the passport of (name of passport owner).

Your kind consideration will be greatly appreciated. Thank you.

OP your title (2Go Delivery for Child's Passport/Visa ) implies you have completed the DS-11 application process, but your statement implies otherwise. If your inquiring about the consent form DS-3053 then read the link below, if not pleases clarify your question.

04/18/2012: St.Luke's Exam Results. SPUTUM TEST REQUIRED for Fiancee. Daughter passes and gets Immunizations

04/27/2012: Sputum Tests NEGATIVE. SLEC still requires Fiancee to complete 4 months of DOT

05/09/2012: Fiancee and daughter move to condo in Manila

05/10/2012: Fiancee's first DOT at St. Luke's

09/09/2012: Fiancee's DOT COMPLETED!

11/14/2012: Second Medical Exams at SLEC. Both Passed and Immunized!

12/10/2012: US Visa Interviews - APPROVED!

OP's timeline/signature says fiancee and fiancee's daughter both passed SLEC medical exams and USEM interviews therefore waiting for their passports with K1/K2 visa.

Daughter is not a US citizen and not OP's bio child.

Sorry for the confusion everyone. Question is not related to a US Passport. To elaborate more...

My Fiance's K1 and her daughter's K2 Visas were approved recently. We are now awaiting delivery of their passports and visas via 2Go Courier service. The website US Travel Docs indicated required IDs upon delivery as well as requirement for children under the age of 16. This "letter of authorization" made no sense to me as her mother (my K1) will be there to receive the 2Go delivery.
