Hi,

So my wife had completed her medical examination and vaccinations in London,England and my question is now that we are married on K1 visa we are on the next step of AOS, do we need to complete a I-693?

You only need the vaccination page completed, if you do not have a copy of immunizations from your medical, or your filing of AOS is more than a year since medical completed for K-1.

I have one more question, I am working on the cover letter for the i-485, do I put my wifes name (from london, england) in the header or my name the US Citizen.

If you had all the required vaccinations at the time of your medical, even if your DS-3025 is marked 'incomplete- may be eligible for blanket waiver', you can skip the civil surgeon and just send a copy of your DS-3025 with your AOS documents. If you still need vaccinations, or had any done after your medical, then you need to see a civil surgeon to get an I-693.

Just to add a minor correction to one of the posts above, even if you do not have a copy of the DS-3025 you still are not required to get a whole new medical if yours was done within a year of filing for AOS. You also would not have to see a civil surgeon for the vaccination transcription onto the I-693 if you are certain you had all the required vaccinations and your DS-3025 was filled out properly because the USCIS will have the original that is with the rest of your medical documents from the sealed envelope you handed over at your POE.

thank you, now i have a concern... my wife had everything checked off on the list of the link you gave... only issue I see is the:

•Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)

which on her ds-3025 says 01.19.2000 and since after her examination there she went to her general practitioner and received all necessary booster shots, TD and TDAP to make her immunizations up to date and we have a printed record from her general practitioner... do we still need a i-693?

By not having an up to date tetanus/diptheria/pertussis shot marked on her record from Knightsbridge, she does not meet the requirements, does she? So the sentence following #4 applies--

If it's not completed like that then see a civil surgeon for the I-693.

USCIS only accepts forms filled out by a panel physician (abroad) or a civil surgeon (US). Her panel physician report is lacking a required shot, so now her option is to get one filled out by a civil surgeon (I-693). The CS will use her DS-3025 and the proof from her doctor to fill out the form and certify she is up to date.

No he will not contact her GP in the UK. She needs to provide the civil surgeon with something signed by her GP naming the shot and the date given. If she didn't get something from the GP, then it's her responsibility to get it sent to her from the UK.

Call around and find the best price. Some people pay $25 to $300 to a CS. They can charge anything they want. Some refuse to do just the shot part and will say she needs the full exam done over. That is not true. Good luck finding somebody reasonable.

How long? Depends on the doctor office. They are in private practice so it depends on how busy they are.
