hello everyone,I was gonna pay for my visa today but the teller told me that theres a form i need to fill out before paying for it.

so anyone can show me what form is it?links pls.?

and what other online forms do we need to have?

03/28/2013: NOA - AOS, AP, EAD
04/25/2013: Biometrics

05/28/2013: RFEs for EADs and APs

06/17/2013: AOS Interviews. Approved

06/26/2013: Green Cards Received! (APs and EAD also received but not needed)

Removal of Conditions

09/07/2015: RFE Response - Additional Evidence Sent

Scroll down and click the Deposit Slip -  $240 MRV Fee, once u were there print it out, u need to click once otherwise they will charge u double LOL!</span><div><span style="font-family: Arial, Helvetica, sans-serif; ">Bring that printed form to BPI. http://ustraveldocs.com/ph/ph-niv-paymentinfo.asp scroll down and click the deposit slip - mrv $240, u need to print that and bring to BPI
