Is the DS 156 from only available in a web version format? I can only find DS 156K in a printable form.

You can print screen and get the form on paper, although it is meant for electronic filing only.

http://www.state.gov/documents/organization/108128.pdf copy and paste this link into google and it will let you open it and print it out and stuff... this is what we used =) good luck
