I can't seem to get a hold of my new tax preparer and it was my mistake that I didn't get a copy of my 1040 for 2011. I will try to get a transcript from IRS tomorrow. Will my W-2, 1 month worth of pay stubs, bank statements and employment letter be enough to serve as supporting documents for I-134? Thanks in advance for any help :)

If your taxes have been filed, you should be able to order a tax transcript online for free and have it delivered by the end of the week (this happened for me). You should at least get that if you don't have the 1040 along with the W2. Everything else looks fine per Manila instructions.

You could send 09 and 10 as well if you want to. I am presenting 3 years tax transcripts, 2011 1040/W2, pay stubs, employer letter, and possibly bank statement. It isn't required to bring the past 3 years, but it shows stability if you've held the same job and/or pay for that duration. It isn't listed on the I-134 form nor the US Embassy Manila instructions, but more doesn't hurt in this case. I ordered all 3 online at the same time, and all 3 came to me at the same time in separate envelopes.
