He doesn't remember anything? A month? a year? really?

When Year/month ... anything

Where The country where it was refused.. assuming Jamaica.

it should be in his passport the date/month or what ever but for the reason, he should be able to put "i dont remember"

I think the critical thing is not to say No if he was denied. He should know the country.

Thanks, I will have him look for his old passport, he probably threw it away. His current passport he renewed in 2004.
