My question is, is your fiance working now? If so, does his income meet the guideline? If it does, he can just fill out I-134 include a statement from employer and recent pay stubs. However if you want to be safe, your cousin should also fill out form I-134, statement from employer and last year tax transcript from IRS (order from IRS website super easy to use). That's pretty much what i sent my fiance for consulate in Bangkok, went to interview and no issue about affidavit of support.

Your fiance MUST fill out the affidavit of support, regardless of whether or not he meets the income requirements. If he does not, then he will need a cosponsor who will also fill out an affidavit of support. Both support forms need to include proof of income, such as pay stubs, employer letters, bank statements, etc.

Is your fiance self-employed? Is he currently working? Need more information to give you the best answer for your situation. Regardless of his income, he will fill out an I-134. Does your cousin live and work in the US? Are they a US citizen or legal permanent resident? If yes, and they make enough for their own household size plus the beneficiary, then they fill out their own separate I-134 and provide 2011 tax transcript, letter from employer, recent pay stubs and proof of being a USC or LPR.
