AR-11 SR form

I'm about to file this form for my change of address, and I had a question around the sections for port of entry and date of entry into US. The last time I was out of country, I returned back to US on 10/2011 and Chicago was my port of entry. I just wanted to know if I'm supposed to enter the info based on the last time I entered US? I just wasn't sure if this section was more suitable for people who've just come to US instead of people like me who've lived in US for a couple of years now. Thanks!

It means the first time you enter the USA, that's what I did when I filed nh AR-11 back in 2010.

I thought it's the last time you enter the US. I chose the entry port that I entered in 2010 to input in AR-11 instead of the one in 2009.

That was my understanding as well, which is why I entered my last US entry date. Did you run into any issues after you submitted the AR-11?

This is what I see on USCIS website when we fill out AR-11:

Last entry into the US

Port of Entry into the US

Approximate date of entry into the US (month/day/year)

If not a Permanent Resident, my stay in the US expires on (month/day/year)

So the correct port entry is the last one, not the 1st.
