All of these things are consulate specific. Generally they require the same fiorms as the K-1 except the DS156K which is K-1 specific. So, if those are the forms you used for your K-1, then yes.

Also consulate specific is the need for a separate affidavit or not. In Ukraine they say "each applicant" needs one, but when we went they looked at both of them, asked me "are these really just the same?" I said "yes" and they detached everything except the I-134 for my wife(fiancee) and handed it all back...including all my supporting evidence. Go figure.

If you are not sure, make two and copy all your evidence. Nothing harmed.

First of all, I would like to thank Gary and Alla for the great information they provided to me when I was inquiring about the same question. I contacted the Guangzhou consulate to find out their specific requirements. They indicated that they wanted a complete, original, signed I-134 for each visa applicant. So, each completed form is different, but the supporting documents are the same.

 Waiting for the interview date, or I would tell you what they did with the forms (as you previously mentioned), affidavits and accompanying paperwork. I suspect they will likely do the same thing as Gary and Alla describe.

Good luck on your visa journey.
