Can anyone share reasons they've heard for visas being denied? Thanks

I've only heard of a couple of visas that were denied at the US Embassy in Bogotá from members of VJ and one was because they couldn't prove that one of the children of the beneficiary was actually his child. If I remember correctly they had said the girl, who was almost a teenager I believe, didn't have a birth certificate so they went to a notary to get a new one a week before their interview. That of course raised a lot of questions and the CO asked for a DNA test which they never took and therefore the visa was denied.

The second case was purely because the CO saw that the relationship wasn't legit, they didn't have enough evidence, they were not convincing enough and couldn't prove what was never there to begin with.
