Does my fiancee need to fill this out even if she wont be my sponsor?

Also, do I need to send both my fiancee's and my sponsor's forms in with Package 3 or can I take the sponsor's one to the interview?

Yes your fiancee still need to fill out I-134.

I am not sure specifically what you send in Packet 3, as it is different by country.

But I would sent both and bring both as well for the interview.

The Petitioner fills out the 134 and sends that with documents such as tax return copies or tax transcripts, paycheck summaries or pay stubs, letter from employer to the Beneficiary for use at their interview.

Yes you need to send both in with your packet along with the documents they as for.
