If extra space is needed to complete any item, attach a continuation sheet, indicate the item number, and date and sign each sheet...per I-134 instructions.

Yep. Reference the attachment sheet on the first line and then list all with the information in order as on the form, on the attached sheet.

Reference the name and case number on the top of the sheet.

Phil (Lockport, near Chicago) and Alla (Lobnya, near Moscow)

As of Dec 7, 2009, now Zero miles apart (literally)!
