Email the actual form? or just send an email in the format available on VJ?

Yes follow the format in the VJ, and it should be the benefeciary who will email it to nvc.

I send an email. I just add the case #. Also how do I know if they have received it?

You will get an email from NVC saying if they've accepted it.

barcode strip not needed, at all.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
