Leave the middle name blank so they do not write your middle name as none or N/A.

Answer other things that do not apply as N/A or None where applicable.

Putting NONE is actually recommended to me by USCIS level 2 rep. That's what i put on mine and it was perfectly fine.

Thanks for the input, i was thinking of just writing N/A, but for question 11, did you put N/A? or none? And for question 17, did you just list yourself as the husband and left the rest of the lines blank?

Thanks :)

Well as long as USCIS doesn't mess it up, we are fine to use NONE or even N/A but there are cases where they did mess things up.

Since USCIS recommends to use NONE, please use that. And when you get NOA1 (acknowledgement), please check the same.
