Question #14- Spouse's middle

I don't have middle name should is put none, N/A or leave blank.

Question #30-List all places you have lived for at least six months since reaching the age of 16

My wife lived there all her life since she was born in 1986, should I put 1986- present

Question# 44- Were you assisted in completing application

I help her fill it out, my question is should i answer yes

08/09/2011: IV fee bill received and paid
08/10/2011: IV fee payment status shows "PAID"
08/11/2011: AOS package accepted
08/13/2011: IV package sent to NVC (Delivered at NVC 8/15)
08/22/2011: IV package accepted
08/22/2011: Case completed (25 days since case# assigned)
08/26/2011: Interview date assigned
09/05/2011: Medical exam (result passed)
10/12/2011: Interview- APPROVED (51 days since case completed)
10/19/2011: Visa pick up
11/22/2011: Arrived Seattle, Washington
12/10/2011: Green card received

Interview took 208 days from I-130 NOA1 date

My wife doesn't have a middle name either, I put dashes for that section.

I interpret this question in it's most literal sense; since you assisted her, then it would be appropriate to say yes.

October 16, 2013: Biometrics Appointment

January 28, 2014: Production of new Green Card ordered

February 3, 2014: New Green Card received; done with USCIS until fall of 2023*

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal
