The documents they are talking about is your visa and immigration package to take with you when you POE. They will ship this to you after your interview.

All information required for the medical exam is on the consulate's website here : http://montreal.usconsulate.gov/content/content.asp?section=visas&document=visa_pkt4

The link in the Medical exam section will provide the list of immunizations and things for you to provide. Once you choose the clinic you want to go to, call them and make an appointment. You can ask them any other questions you have about the medical examination.

They will do a 5-10 minutes appointment with a doctor, take a chest x-ray, draw a blood sample, and check your immunizations. You must declare all major/chronic illnesses (including mental health), provide drugs & medication history and provide prescription for any drugs you might be taking. Should cost you around 250$ CAD, it takes about an hour and it will take at least 3 days before you get your results.

The consulate does not send you any mail after Packet 4 which is a letter saying you can make an appointment and duplicates the info displayed on the consulate's website. You must take this letter to your medical appointive AND to your interview along with the CSC confirmation page showing the date and time. The rest is all listed on the link I provided above.

Good luck!
