what should we write in the empty line

and i never visit US .....

I mean to say...I will put never in the spaces it says put never (per the instructions n form) if not visited usa but in all the other blank spaces do I put None and N/A where necessary?

I think just one NEVER is good enough. Never assumes N/A for all the other blank spaces.
