she will need the DS-156 & 157, but will not need the DS-156k. she will not need an NBI Clearance since she is under 16 years old.

When you are ready to get your CFO sticker (meaning hawak nyo na ang visa ninyong dalawa/visa is in hand for both of you), you just have to register her at the CFO. May form sila that you will fill up (There is a form you have to fill up). Kung ano ang fee mo for the sticker, yun din ang fee niya. (Whatever your fee is for the sticker, that is also the same fee you will pay for hers.)
