Where can I find the "Information releaseagreement form"

My application has exceeded 5 months mark. I contacted congressman yesterday. She was aking about the 'information release agreement form". Where can I find that form and what other document I have to include? Thanks in advance

If your congressperson has a local office, you can drop in there. They can provide it to you, and have you sign it on the spot. And that might help you as well, because the staff there can really get there teeth into USCIS. :hehe:
