Mysterious Medical Forms

One of the problems I am having is, at no point during the K1 process did any of us know what the mysterious "medical forms" were called. They were all sealed in a super-secret envelope that we were not allowed to open. Now I am being asked to produce these forms, and no where in any of the guides are these forms addressed specifically by number. They are not even listed on the list of items to bring to the K1 interview in Montreal. Yet, "Officer Smith" of USCIS actually asked me if I had a copy of these forms. Why is it USCIS does not know their own procedure?

So how do I explain to my Congressman that I am certain that these forms were submitted, when I never saw them myself, don't have a copy, and can't find them specifically listed anywhere?

The forms I am being asked for are:

DS- 2053

DS- 3024

DS- 3026

Where are these in the guides? Nowhere.

When do we see these forms? Never.

Why are we responsible for collecting these forms when they are not allowed in our possession at any time?

Contact the original doctor who did the K-1 medical and see if they will send you a copy of what they filled out for you. They must have one in their records somewhere.

Thanks. I did contact the original doctor. I am waiting to see if they have them.

I found those links too but noticed the forms seem old (2007). I can't find the forms on the USCIS website to see if there are more current versions. I don't want to give them to a CS only to find out they have expired.

I contacted the CS in Canada, the US Consulate in Canada, and the BP at Lewiston, NY to see if any of them have the forms. The BP told me they do not have them.

I made an appointment at a CS in my area for Nov. 9th, because the forms are due Nov. 27th.

I am going to contact my congressman on Wed., but this post was a vent of my frustration at this part of the process.

Only the 3024 is an old form, the others are Sept 2010.

Good luck, I hope you manage to get an answer. This situation is a bit ridiculous....

Man Arabella, what a ridiculous situation. All because someone, somewhere along the long doesn't know how to keep paperwork together?

I remember someone on here contacting Dr. Seiden for a copy of their vaccination record. If I remember correctly their request was declined for no good reason. I want to say it was Collens? Maybe? Point being, I doubt Dr. Seiden would give you a copy. I hope I'm wrong.

If it were me (and I know it's not) I would probably just throw in the towel and have a new medical done. It is another silly fee, but I bet it would be quicker and less of a headache for you.

i asked dr. seiden twice for a copy of the paperwork and was told "under no circumstances can they reveal to me what is in the sealed envelope" which meant getting a copy just in case was impossible. i even asked before i left , casually where my copy was. and was again told..i dont get one...and they are not allowed to give me one..:( which i dont get..it's my medical test my history so what the eff gives??

from what i have read, this basically means they lost your medical somewhere between POE and UScis. however how we prove this i have no idea, as there is no signature required when we hand over the paperwork at the border, but it is a requirement for us to process our k1, for them to let us in the states. this is sooo frustrating and i'm so sorry for this. but it is a worry i've thought about for a while with me and colleens delay as well.

Well that's good to know. I am going to download and pre-fill out these forms before his appointment on Tuesday.

We are. I made an appointment about an hour after reading the POE for a repeat medical (see my timeline). It's a very reasonable price ($120).

I think that's right. I called the Lewiston Bridge POE yesterday (I got the number from USCIS) and I was told that they NEVER take the medical unless you are an immigrant Visa. They told me that K1s are nonimmigrants, and therefore they don't take the medical.

The only problerm with that is . . . they did take it! They are supposed to take it! They don't even know their own procedures!

Sadly - it will probably be less headache just to get it redone for that price.

You know - you'd think they would put 2 and 2 together - no way was your K1 approved without the medical so that should make them wonder where the paperwork went!

And another thing - why are those records private and sealed after the fact? It was your medical which you had to pay for outright - such #######.

I don't know if you care Arabella - all you need to explain is that your hubster handed over his pkg of papers at POE when he activated his VISA. They're the ones who are supposed to know.

I would guess that they image everything (providing they don't lose it first of course!) so that any office can access your file.

oh thats fantastic! when was your medical? just wondering as they told me they do not keep copies and mine was back in January. wondering if they changed things since. oops nvm i see seans was in november. at least if you get them you'll have copies for his immigration file at home now!
