This is odd. Did the civil office even guide you to where you can obtain it. Where do the records go after 20 years. Makes no sense to me.

10.19.2012 Check cashed (it never ceases to amaze me how fast the gov't cashes a check!!)

My would be spouse doesn't have a birth certificate. Here birth record is also not available any more as civil office keep records only for 20 years. Can someone give me a template that I can use to submit in place of birth certificate?

Give us a hint here, what country are we talking about?

She is from India. In India its not so uncommon as earlier people are not required to register birth of the kids by law. I remember even when one of my colegue filed for his GC he got some certificate from local Civil office stating " No Record for So and So person is available" and then he got some Affidavit from his parents (I guess) on Indian stamp paper giving details of parents name , birth place, birth date etc.

I just want to know if there is any template with all the details needed by NVC when such document is submitted.

I remember even when one of my colegue filed for his GC he got some certificate from local Civil office stating " No Record for So and So person is available" and then he got some Affidavit from his parents (I guess) on Indian stamp paper giving details of parents name , birth place, birth date etc.

I just want to know if there is any template with all the details needed by NVC when such document is submitted.

Your options:

1) Sometime in the past (in 2000), late registration was possible in India. I did do. Check this option is still there.

2) If option# 1 is not there, follow the below.

First, get an official record from the birth registrar that the "birth is not registered in their records".

Second, make 2 notarized affidavits from the applicant's parents on the stamp paper.

Along with the above, produce a school certificate that shows applicant's name, DOB and parents' name.

You will need 2 things from her, One Affidavit from her parents that she was born on so and so date, take this affidavit to local municipal office and they will issue a Non Availability certificate indicating that her birth was never recorded and is not available, I hope this help.
