Hi everyone. I am new here. I am in Long Island on a K1 visa and we got married yesterday (October 7). I only have until November 20 to file for the AOS.

Unfortunately, I was compiling my papers needed for the AOS, I just found out that I lost the Vaccination Documentary Worksheet given to me by the St. Luke Medical Center in Ermita. I tried all options to locate it but I can't find it anymore. Should I request another copy from St. Luke? Anyone who have any direct contact person in the request section there?

Welcome to America and congrats on your wedding :) First there is no deadline to file AOS. You have 90 days to marry from the time you arrived, which you have done. It's best to get a copy of your vaccinations if possible otherwise you might have to take a full medical here. Best of luck to you.

09/2009 - NOA 2 approved

4/26/2010-Received texts and emails 7th day

4/30/2010-Received NOA's(Hardcopies) 11th day

5/3/2010-Received ASC appointment notice(mailed 4/29/2010)14th day

5/7/2010-Walk-in Biometrics done(2 weeks earlier)18th day

5/13/2010-Case transferred to CSC

6/2/2010- Case received/resumed at CSC

6/18,6/22,6/23 AOS touches

6/28/2010- EAD production and touch on AP

6/29/2010-AOS APPROVED

7/2/2010- 2nd update on EAD production and touched on AP....

7/6/2010- Received "Welcome Letter" and AP document

7/12/2010-Received GREEN CARD and EAD

Welcome to the States and congratz on your marriage! :star:

If I were in your shoes, I would submit your adjustment of status (AOS) papers right now, even without submitting your transcribed vaccination record by a civil surgeon. I forget which form it is...I think it's the I-693.

Then, if you receive a request for evidence (RFE) regarding the I-693, you could simply send it in. This will give you more time to get a replacement record from St. Lukes without having to delay your AOS.

Keep in mind that others here on VJ have neglected to send in their transcribed vaccination record and they never received an RFE. So, it's entirely possible that you can get your green card, your advanced parole (AP), and your work permit (EAD) without having to send in your I-693.

What I would not do is delay filing for your AOS until after your I-94 expired. You could run into problems if you were caught while you were out of status. Why take a chance? Just send in your AOS papers.

Congratulations on your marriage. We filed my wife's AOS without including the DS3025 or I693. Went through with no problems or RFE's. The medical info is included in the packet that you hand in at POE. As others have siad it is best to get it submitted. Any questions PM us we're in Smithtown. (LI)
